Dr. Jenkins has been performing FAA physical exams for patients in the Rock Hill and Charlotte area since 2005.  He performs Class I, II and III aviation exams along with Air Traffic Controller Exams.  Dr. Jenkins is HIMS certified for cases of substance abuse and SSRI use.

As of April 1st, 2012, all pilots are required to log in to https://medxpress.faa.gov and fill out their portion of the 8500-8 form.  This must be completed and a copy must be printed and brought to your appointment along with your confirmation code.  This code is necessary to access your file online.

If you have been given a special issuance in the past, please be sure to bring this paperwork to your visit.

You are advised to bring all necessary information from your outside physicians so that your certificate may be issued at the time of service.

FAA Physical Points to Remember:

• Please arrive 30 minutes prior to your appointment time to complete paperwork.
• We do not file any insurance for flight physicals.
• The cost of a flight physical is $125.00
• For Class I physicals that require an EKG, there is an additional $50 charge.
• Payment is expected at time of service
• We accept cash, VISA, Discover, or Mastercard
• There may be an additional charge for same day work-in appointments.

ATCS Exam Points to Remember:

• Please arrive 30 minutes prior to your appointment time to complete paperwork.
• If you require a First Class Airman Medical Certificate at the time of the visit, additional fees may apply.
• If you wear contact lenses, please remove them prior to your appointment and bring them with you so that we can check visual acuity with and without correction.
• If you wear glasses, please bring them with you to the visit.
• Make sure you have Form 3930-4 with you or the appointment will be rescheduled.

• There will be a minimum of 2 visits for this type of exam.
