Insurance

• We are contracted with Medicare, Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, Medcost, and many others.
• Please present you insurance card at each visit to help us keep your information current. If we do not have your current insurance information, you will be responsible for the bill.
• If we are not contracted with your insurance, we can file insurance after payment in full has been made. A twenty-five percent discount is given if visit is paid in full at time of service.
• WE CURRENTLY DO NOT ACCEPT MEDICAID.

Payments

• Accepted forms of payment are: Cash, MasterCard, Visa, Discover or Check.
• We ask that deductibles and/or copayments be paid at time of service.
• If you have no insurance, a twenty-five percent discount is given when payment is made in full at time of service.
• If you find that you are unable to pay for your visit or balance, please contact our Business Office so that special arrangements can be made.
